    Job Description
    Would you like to work with young parents? Family & Children's Service, Chelsea, seeks a bilingual/bi-cultural Home Visitor to work in our Healthy Families Program.

    Full time flexible hours required for Family and Children’s Service of Greater Lynn, Healthy Families Harbor Area program serving the towns of Charlestown, Chelsea, East Boston, Revere, and Winthrop. Exciting opportunity to work at Healthy Families-Harbor Area. The Healthy Families Home Visitor will provide support services, health and parenting education, child development assessments, and referrals for first time parents, 20 years or younger starting at pregnancy and continuing until the child is three years old.
    • Work with other staff to ensure that participants access healthcare and receive family planning information.
    • Participate in individual and group supervision, and agency’s staff meetings as required.
    • Tracking, recording and maintaining patient data in the Participant Data System (PDS) and implementing all appropriate evaluation tools as instructed.
    • Perform developmental screening on children using Ages and Stages Questionnaire (ASQ and ASQ/SE).
    • Developing individualized Family Service Plan that addresses the family’s specific needs.
    • Offering maternal/child health education to program participants.
    • Engaging and developing transformation and teaching relationships with pregnant and parenting participants of different cultural/ethnic backgrounds.
    • Understanding, practicing, and promoting the Healthy Families Massachusetts (HFM) mission, values, methods, and model.
    • Accepting referrals of new and/or transferred HF participants.
    • Maintain weekly, bi-weekly, and monthly home visit service level standards as required by (HFM), including outreach, engagement, and re-engagement activities.
    • Counseling parent on achieving personal goals like returning back to school or securing a job.
    • Completing timely evaluations, assessments, and referral forms in order to ensure accurate delivery of services
    • Encourage participating fathers/father figures, co-parents to participate in program activities including parent groups and fathers groups.
    • Developing and maintaining participant hard copy files with appropriate documentation such as signed consent forms and other forms as necessary.
    • Completes all mandatory HF trainings and prerequisite internal trainings.
    • Create and maintain participant files manually and electronically according to the HF standards.
